Cancellation Policy
We have the most dynamic and balanced group of highly skilled individuals who are ready to serve you. We believe that our commitment to your time and personalized care are some of the reasons you are here at Hamden Family Dental. We pledge to provide you with the highest standard of care. In turn, we require at least 48 hours notice if you are unable to keep your appointment, or we reserve the right to charge a $75 missed appointment fee. This advance notice will allow us to make arrangements for other patients that could use that time slot. Your cooperation in this matter will help us maintain a fair and competitive operating cost. Weekends and holidays are not considered part of the 48 hour notice.

Payment Options
Payment including all insurance co-pays is due at time of service.

In addition to cash, personal or bank checks, and all major credit cards, we offer the CareCredit payment plan option. The CareCredit plan can be quickly and easily accessed at our office or from your home.

Insurance Plans
We will gladly process claims for all major dental carriers. Due to ever changing insurance regulations, please call our office for our current participation.
